Shipping Documents
BILL OF LADING, AIRWAY BILL must include the following:
- Ship to address, exactly as it is on the shipping instructions
- Notify Party, as provided on the shipping instructions
- Ocean, Place of Delivery (City and State)
- Air, Airport of Destination
- The third party Carestream Healthcare International bill to address, when applicable.
COMMERCIAL INVOICE to accompany shipment, is essential to complete Customs Clearance and must include the following:
- Seller name and address
- Supplier (manufacturer of goods)
- Delivery Address with building number contact name at Carestream Healthcare International and phone number
- Purchase order number
- Quantity
- Complete description of product (in English)
- HS code (Harmonized System Code)
- Catalog number or Carestream Health part number
- CIN# for chemical products
- Country of origin (Country of manufacture)
- Value of goods with correct currency
- Gross and net weights for each item
- Terms of Sale (Incoterms 2000)
Packing List
- Each Carestream Healthcare International Purchase Order Number must have a separate Packing List.
- Purchase Order Line Item Number, (for example, 01, 02, etc.)
- Weight associated with each Purchase Order Line Item
- Mark the total number of cartons shipped on Packing Lists(s)
- Packing Lists must accompany the Commercial Invoice.
Marking
- Mark the outside of each shipping package with the Carestream Healthcare International Purchase Order Number(s)
- Mark the total number of colli shipped on the outside of each box (example: 1 of 6, 2 of 6, etc.)
- Mark the outside of each shipping box with the Country of Origin (Country of Manufacture) Customs will delay any cartons not marked.
- Mark the complete delivery address on the outside of each box.
Packing requirement
- The supplier should provide products in packaging suitable for export shipment, they have to use adequate cushioning and dunnage materials to prevent damage from shock and vibration during shipment. The amount of protection required depends upon the fragility of the product and varies by the protective materials and mode of transport used. Cushion small products within their packages. Medium and large products can be protected with wooden materials or special isolation systems.
- Unless otherwise specified, all finished goods shall be shipped in individual packs using single containers. These items must be packaged in a manner that eliminates the need for repackaging prior to shipment to customer sites.
Since finished goods are frequently reshipped individually, supplier packaging must:
- Be suitable for direct shipment to the end customer.
- Provide adequate protection against rough handling typically encountered during express, parcel, and courier transportation.
- Maintain product integrity throughout multiple handling stages and transportation cycles.
- Ensure the product arrives at the customer site free from damage and ready for use or installation.
The supplier is responsible for selecting packaging materials and designs that provide sufficient protection throughout the entire distribution process.
Insurance
- Do not purchase or sign for additional insurance or declare a value beyond the carrier's normal liability. Carestream Healthcare International will not pay for additional insurance.
